HealthScreen Disease Management is a health risk management company who specializes in population plan management for self-insured employers and now individual patients. Unique in the healthcare industry we utilize active monthly management by creating a personal relationship between one of our highly skilled nurses and the enrollee. Our programs have been proven highly effective in managing both common and chronically ill disease states.
We feel that managing a disease state starts with creating a disciplined structure for our enrollee to help with any behavioral and /or clinical improvement. Each of our enrollee's start with stage one that includes Active Monthly Management with one-on-one support from one of our care nurses. As the enrollee progresses and shows improvement in his or her disease state, they will also progress through our program with more knowledge and self preservation.
